Accessing Services: It starts with a referral

Setting up PT services

The staff of Abilene Physical Therapy are ready to help you access the care that you need. This usually begins with a diagnosis of an illness or injury and a doctor's recommendation that physical therapy would be a beneficial part of your treatment plan.

You, the patient, always have a choice of where to receive your physical therapy services. When consulting with your doctor, ask him or her for a physical therapy referral to our clinic. APT has provided referral forms to physicians' offices.

A referral is almost like a prescription -- it indicates the diagnosis, the frequency of therapy sessions per week and the types of therapy your doctor wants you to receive. It also gives the therapist the directive to "evaluate and treat," which means the therapist has flexibility in developing your treatment plan. You need to bring this referral form to your first appointment at APT.

If you have not yet discussed physical therapy with your doctor for a diagnosed injury or illness, you can call us at 695-6011 for information about starting therapy. If you have consulted a doctor recently, we may be able to call the doctor's office and request a referral for you.

A doctor's referral is necessary for insurance companies to approve physical therapy benefits.

Our Health Care Partners

Abilene Physical Therapy provides at-home physical therapy services through Outreach Health Services, 1665 Antilley Road, Suite 100, and Caprock Home Health Services, 500 Chestnut St. Patients who qualify for home health may speak with the appropriate agency about utilizing our services.

We also work with several medical supply companies in Abilene, including Owen Pharmacy, 2125 S. 20th St.; Affinity Medical Supply, 1602 Hickory St.; and Choice Medical Supply, 17 Windmill Circle.

Insurance
Abilene Physical Therapy accepts Medicare, Medicaid, most private insurance plans, and private payments. We will verify your insurance information and coverage and provide you with an estimate of your financial responsibility, if any.

It is always a good idea for you to call your insurance provider to clarify your benefits, particularly your co-pays and the length of treatment that will be covered.


Location and Hours
Abilene Physical Therapy is located on South Danville (frontage road) a few blocks from the intersection with Buffalo Gap Road. Our office is in the Fed Ex Office (formerly Kinko's) plaza.

Our office hours are:
Monday-Thursday from 7 a.m.-Noon and 1-5 p.m.
Friday from 7 a.m.-Noon.
